Service Description

Medical cannabis has been available in Arizona since 2012 to treat a variety of qualifying conditions. In Arizona, the qualifying conditions for cannabis use include ALS, Alzheimer's, Cancer, Crohn's, Hep C, HIV/AIDS, Glaucoma, PTSD, wasting syndrome, severe nausea, seizures, severe muscle spasms and severe/chronic pain. If your qualifying condition is PTSD, you will be required to provide evidence that you are currently undergoing conventional treatment. If you have medical records pertaining to your qualifying condition, they can often help you along your journey to certification. After a doctor confirms that you qualify for the program and signs your certification form, the paperwork will need to be filed with the state of Arizona. You will be required to pay the Arizona state registration fee in order to receive your MMJ card. The state fee is an additional $150 unless you receive food stamps. If you can provide proof that you receive food stamps in the state of Arizona, the state fee will be reduced to $75. If your food stamp card has your name printed on the front, that will prove you receive food stamps. If it does not have your name printed on the front, you will need to provide your "award letter". We can help you file your paperwork with the state of Arizona. Our full service certification includes filing of your paperwork. If you choose "cert only" you will need to upload your own paperwork. If you would like to file your own paperwork, that is an option. You will need to upload a passport style photo of yourself, a copy of your driver's license, your signed attestation form and a copy of your doctors certification to your Arizona state portal. After all of the documents are uploaded, pay the state fee and wait for an email from AZ DHS (Department of Health Services). If you file your own paperwork you will be responsible for correcting any mistakes on your application. If mistakes are not corrected in a timely manner, you may not receive your card.
